Library card application requirements
Two forms of street address identification are required to get a card at the Wallkill Public Library. One of the forms of ID must be a picture identification. Acceptable forms of ID must show a patron's current physical street address. These may include: a piece of mail, driver's license, tax bill, bank statement, utility bill, rent receipt or apartment lease. At the time of application, for children entering kindergarten up to age 12, a parent or legal guardian must be present with their own ID. No adult signature is required for children 13 and up.
Library cards are free, but replacements for lost cards are $3 each.
Your card gives you free on-site and online access to all 47 RCLS libraries.
Only cards for the Wallkill Public Library will be registered via this form, as we cannot process library cards for other libraries. An exception will be made for Plattekill patrons who can get a courtesy card to use at Wallkill, as well.
Library card applications received via this form will be processed during normal operating hours, and mailed out to the registrants via U.S. Postal Service mail. If you receive mail through a P.O. Box please include that information with your address.
